We're excited to welcome your family to the NTX Renegades for the Fall 2026 season!

This form helps us :

Get to know your player as an individual.
Coordinate uniforms and player information.
Collect walk-up music and other team details.
Better understand how we can support your player throughout the season.
Talk about what goes on behind the scenes to make a season successful for everyone involved.
Prepare for practices, games, and team communications.
Please complete one form for each player.
The questionnaire should take approximately 8-10 minutes to complete.
